£24,970 - £25,183 per annum (pro rata for part time staff)

The Rapid Response service are looking for Rapid Response Assistants to work in a multi-disciplinary team of professionals working in delivering a 24hr response service to avoid hospital admission and to facilitate hospital discharges, you would carry out person centred care offering practical support and advice.
This is a developing service with great career prospects and training on offer, you must hold a Valid driving license and have access to a car.
This is a great opportunity for someone to take their career further within the health and social care and join an amazing team.
The service is open Monday to Sunday 8am till 10pm 365 days including Bank Holidays.
