Brent’s diversity is evident to all who visit our borough and our long history of ethnic and cultural diversity has created a place that is truly unique and valued by those who live and work here. <br><br>The council is pursuing a far-reaching transformation agenda with our partners that better meets the needs of our community so it is an exciting time to join us. <br> </p>\n<div><strong>The Post</strong></div>\n<p>The role will be to lead projects and contribute to the delivery of programmes across adult social care and health working collaboratively with our Community Service Provider and other Integrated Care Partners in Brent.</p>\n<p>The post holder will be expected to work closely with local care home providers and key stakeholders to develop and align with the ambitions of the Enhanced Heath in Care Homes and social care to support people living in care homes.  </p>\n<p>In addition, the post holder will be supporting the Integration and Delivery Team with managing a variety of complex and exciting community healthcare contracts to develop and improve healthcare services.
