The TRI team are responsible for providing a therapist led reablement service that supports people as part of the Adult Social Care department.<br><br>In this role, you will complete assessments and work collaboratively with people who use our service as well as others in the multi-disciplinary team. Identifying meaningful goals and outcomes as well as equipment, therapy interventions and reablement visits to deliver a short term (usually up to 6 weeks) service. The Role
