This includes assessing individuals and arranging practical and social support for vulnerable people to facilitate a safe and timely discharge from hospital. </p>\n<p>You will manage a defined case load delivering high quality assessments and support plans that address the needs of vulnerable people through the commissioning of individual packages of care, whilst upper holding the excellent reputation of the Council. </p>\n<p>We will offer robust supervision, opportunities for continuous professional development and a firm commitment to your personal development.
