This will include providing assessments to those recently discharged from hospital, those who are at risk of admission to hospital and those in rehabilitation units.  </p><p>Collaborate with individuals, families, and carers, adhering to statutory legislation and embedding strengths and relationship-based practice in assessments.</p><p>Working collaboratively and effectively with the multi-disciplinary team to achieve a safe and timely discharge from post hospital care services, whilst promoting and upholding social work values.   </p><p>Demonstrate confidence in safeguarding vulnerable adults and assessing risks, leading on s42 enquiries. </p><p>Understand the critical role of information and advice in promoting independence and wellbeing.</p><p><strong>Our Work Environment:</strong></p><p>We work in a ‘hybrid’ fashion, with our office base at Hove Town Hall and working from home, fitting this around community assessments. </p><p>We have built strong working relationships with our health colleagues.
