These three functions are:</p><ul>\n<li>Wellbeing Advisor</li>\n<li>Multi-Disciplinary Team (MDT)</li>\n<li>Safeguarding Triage</li>\n</ul><p>All three functions will focus on a seamless person-centred, strength-based approach from first contact. In practice, Wellbeing Advisors, Social Workers, Social Care Assessors and Occupational Therapy will work together as a team. The aim being to prevent, reduce and delay people needing formal and longer-term care, and to provide short term reablement support, where required, to enable people’s independence.</p><p><strong>We are recruiting 2 Senior Practitioners to the Safeguarding Team and the MDT Team.
