The Role:
The successful candidates will join City-Wide Mental Health Assessment and Care Delivery to assist providing interventions to adults who have a wide range of care and support needs arising from difficulties with their Mental Health.
You will ensure a strengths-based approach is implemented in the completion Care Act assessments and reviews, in the planning of s117 aftercare services, and, where appropriate, carrying out s42 (safeguarding) enquiries.
You will co-produce support plans that will increase inclusivity, collaboration, and innovation, leading to more sustainable solutions.
The posts are currently co-located with health partners across a number of sites throughout the city.
