An exciting opportunity has arisen in the Adult Social Care Service within Sefton Council for a registered social worker to join the North area team, based at Magdalen House
The role
You will be working in a Triage Assessment adult social care team, providing a service to residents with a variety of needs. You will complete assessments of social care needs and developing support plans for individuals; complete assessments for NHS Continuing Healthcare; provide welfare rights support; support hospital discharges and attend a variety of different multi-disciplinary team meetings. You will be a motivated, enthusiastic, and experienced social worker, with a range of skills and a desire to work in adult social care. You will be based at Magdalen House Bootle, with the opportunity to work in a hybrid manner, and you will receive regular professional supervision and have access to a range of training and development opportunities. If you are newly qualified, you will be supported to complete the requirements of the ASYE programme.
