You will undertake a holistic assessment of need to identify person-centred outcomes.<br><br>We work in an enabling way with people and their families and carers, to promote independence and participation, through meaningful occupation and a strengths-based approach. Across all our services we aim to ensure that people are provided with the right support and advice, and at the right time.<br><br>Our Adult Care Services Connected Lives practice principles are very much aligned to core occupational therapy values, with the ultimate aim of connecting people to the life that they want to live.<br><br>We work collaboratively alongside social care colleagues, as well as with our health partners, housing and care providers, and a range of community support services.<br><br>The role will also include an opportunity to mentor and develop others.<br><br><b>About you</b><br><br>You will need:<ul>\n<li>A recognised occupational therapy qualification and be registered with The Health and Care Professions Council</li>\n<li>To ensure practice is consistent with The Royal College of Occupational Therapists' Professional standards for occupational therapy practice, conduct and ethics, and the HCPC Standards of Proficiency for Occupational therapy</li>\n<li>Knowledge and experience of relevant social care legislation, such as the Care Act 2014, Mental Capacity Act 2005</li>\n<li>To be a confident team member with good decision-making skills</li>\n<li>The ability to work successfully with partners and other agencies</li>\n<li>A full UK driving licence and regular access to a vehicle for work purposes</li>\n</ul><br><b> </b><br><br><strong> These are ongoing vacancies and applications will be considered as and when received. </strong><br><br><strong> For an informal discussion and more information please contact: sheryl.jeram@hertfordshire.gov.uk </strong><br><br>This job role is within the , level job profile.
