We have an excellent opportunity available for a Social Care Practitioner to join our friendly older adult social care team at Wokingham Borough Council, on a full time, permanent basis.
As Social Care Practitioner you will work under the supervision of a registered practitioner to undertake direct work with individuals and their carers. You will use a strength-based approach to maximise individual’s abilities to live independently as possible and remain safe in their community.
Part of this role will include supporting carers to continue in their caring role for as long as they are willing and able to do so.

Key responsibilities:
- To visit and engage with older adults with functional and organic mental health conditions and assess care needs as required
- To meet and support carers and complete carers assessments
- To support people to identify and engage with appropriate service providers
- To work with partner colleagues in the LA, health and voluntary sector
Candidate requirements:
- Relevant NVQ qualification or willing to achieve
- Experience of working with vulnerable adults
- Experience of working in a health or social care setting/ or in a customer facing role
- Excellent verbal and written communication skills – including ability to engage with individuals with a diverse range of communication needs
- GCSE Maths and English or equivalent
This is an excellent opportunity to develop and advance to a Grade 6 once all progression criteria's have been met.
