This is an exciting, challenging and rewarding opportunity to join our Hospital Discharge Team, based in Whiston Hospital.
We are looking for an enthusiastic, motivated and proactive person to join our team, to provide assessments for people who are identified as having social care needs and requiring support on discharge from hospital.
As a Community Care worker you will undertake social care assessments and capacity assessments and work closely with health colleagues and provider agencies.

Applicants should have experience of working in a social care setting. The ability to develop supportive relationships, promoting independence and well-being are key for this role. You will be highly motivated and have experience of working as part of a team, the successful applicant will need to have good interpersonal and communication skills, and be able to work well with colleagues and partner agencies, be flexible in approach, and be willing to continually improve their own practice and service provision.
You must be willing and able to travel across the Borough.
