If you have ideas, grit and a desire to improve people’s lives, Newham is the best next step for your career.</p> <p><strong>About The Role</strong></p> <p>Working with a variety of stakeholders, you will transform services and deliver positive outcomes for Newham’s Care Act eligible residents - putting in place safe, high-quality, value-for-money services that enable residents to improve their health and wellbeing, safe in their own home.  </p> <p>You will be responsible for:</p> <ul>\n<li>embedding our to be newly commissioned Reablement Service and establishing an outcomes focused monitoring approach that will maximise resident independence and value for money (including the development of incentive based payment approaches);</li> <li>working in partnership with our Occupational Therapy Team to manage the contracts we have in place for home adaptations;</li> <li>contract managing our Carers Support Service - and working with our Transformation Team to enhance support to unpaid Carers (including developing the Respite Market and establishing a Carers Digital Support offer);</li> <li>contract managing our Home and Settle Service – supporting residents to settle back into their home and normal routine following discharge from hospital;</li> <li>reviewing and improving our approach to Concessionary Travel.</li> </ul> <p><strong>About You</strong></p> <p>You will have:</p> <ul>\n<li>knowledge and experience of Adult Social Care;</li> <li>experience in all aspects of the commissioning cycle and be able to critically appraise service delivery to develop innovative solutions;</li> <li>experience of writing and presenting detailed and well-reasoned reports and business cases to senior management boards;</li> <li>the ability to contract manage and establish manage effective working relationships with Council and Health colleagues, local Providers and residents;</li> <li>strong organisational / ’political’ awareness and record of successfully delivering and influencing organisational change.</li> </ul> <p>We are keen to work with exceptional individuals who have the experience, vision and ambition to help us create an improved and sustainable future for our Care Act eligible residents and those on the edge of care.
