Your role will also support colleagues with advice, guidance and training.<br><br>\n</div><div>Your expertise will be instrumental in managing a robust social care needs assessment process that meets all statutory requirements. By working closely with existing Social Care and SEND Teams, you will ensure seamless planning for children with SEND, particularly those with Education, Health, and Care Plans (EHCPs). The role will also have a reach into adult social care as we seek to improve our pathways for children and young people into Adult Services.</div><div><strong>About you</strong></div><div>We are looking for committed and compassionate individuals who are entrusted to making a difference.
