HDAT provides adult social care support, in line with the Care Act 2014 and with regard to the Mental Capacity Act 2005, to residents of North Somerset who have ongoing social care needs and are being discharged from hospital after an in-patient stay. We follow the Hospital Discharge and Community Support guidance.<br><br>The team consists of Senior Social Workers, Social Workers, Adult Social Care Workers and admin support. Staff carry out assessments in line with the local authority's statutory responsibilities. Staff identify eligible needs and produce care and support plans to ensure a person's needs are met effectively for the longer term. Staff advocate for the individual whilst working positively with family members and carers, together with other professionals involved with the person, for the best outcomes. Staff work proactively in a strengths-based way to ensure people are also able to draw on their own abilities and support networks for a quality of life, in addition to care provision. The team works hard in a busy environment to support timely discharge with minimal days lost.<br><br>Our team follows the mobile worker model, sharing desk time between home and our office base at the Town Hall, Weston-super-Mare. Most assessment work is carried out in the community in care homes or in people's own homes. Some of your work will be hospital based, supporting the \"Home First\" model in the Transfer of Care Hubs to facilitate effective discharges and to assess where needs are particularly complex and unable to be supported through the usual health D2A routes. <br><br>HDAT prides itself in its commitment to the service users and the service. We are proactive and take a whole team approach for decisions and developments, and actively support staff development in learning and career progression. <br><br><strong>This position is exempt under the provisions of the Rehabilitation of Offenders Act (Exceptions) Order 1975 and is subject to an enhanced Disclosure and Barring Service check. </strong><br><br><strong>Valid driving licence and own transport, or be able to provide alternative, suitable method of travel is an essential condition for this role.</strong><br><br><strong>About You</strong> <br> <br>We are looking for a qualified and Social Work England registered Social Worker who is passionate about their work, and who possesses the skills, knowledge and abilities to undertake this role. You should be able to work effectively with people and their families in difficult circumstances, and be able to guide support and guide them through unfamiliar processes. The Role
