We are seen as a flagship service, especially for the work that we do with the Sensory Strategy , influencing and shaping, keeping people with sensory needs at the heart of our work, working closely with partners. Sensory Services is mentioned in our recent Care Quality Commission (CQC) where Hertfordshire County Council's Adult Care Services is rated \"Good\" overall.<br><br>We are proud to have a fully inclusive, diverse team of VRS, VRS apprentices, community care officers (CCO), social worker and deafblind workers. We use assessment tools and support plans for the people we support and their carers to carry out assessments in the person's own home.<br><br>We provide rehabilitation input using an outcome focused approach including provision of low vision aids, rehabilitation training in orientation, mobility, and daily living skills.<br><br>We provide additional in-house training to enable you to support people for example, short courses on specific conditions e.g. dementia/ complex needs, knowledge of Safeguarding/ Mental capacity Act and functional assessments (such as providing rails or other equipment)<br><br><b>About the role</b><br><br><strong>What you'll do</strong><ul>\n<li>Work collaboratively and build professional trusting relationships with people with sensory needs, their carers', and other organisations.</li>\n<li>Provide expert information, effectively risk management advice and guidance in complex situations to improve the customer journey experience.</li>\n<li>Deliver tailored packages of Rehabilitation around daily living skills and mobility using a person centred, strengths based approach and creative personalised efficiency solutions to assist people to manage their lives independently.</li>\n<li>Ensure the safe use of a wide range of specialist equipment with an up-to-date knowledge of: eye conditions and treatments, along with knowledge of the practical application of teaching and learning theories.</li>\n<li>Undertake assessments and complete reviews to ensure that outcomes identified at assessment are met through the provision of support, equipment, and enabling programmes.</li>\n<li>Hold a small caseload of complex cases.</li>\n<li>Provide professional leadership, training on peer development and practice, support and supervise staff to develop their skills and knowledge in line with related legislation including the Care Act, and Hertfordshire's Connected Lives principles to contribute to the ongoing development of the service.</li>\n<li>Actively promote Equality and Diversity and challenge discrimination.</li>\n</ul><br><b>About you</b><br><br><strong>Essential</strong><ul>\n<li>Qualification i.e.: Diploma/ Foundation degree in Rehabilitation studies (Visual Impairment),</li>\n<li>Significant experience of working with adults and children with visual impairment including those with additional disabilities/ support needs</li>\n<li>Registered with Rehabilitation Professional Workers Network and/or Habilitation VI UK (formerly MISE).</li>\n<li>Excellent verbal and written communication skills</li>\n<li>Understanding of the Care Act, and other statutory legislations within the Adult Social Care and Children's Services, relevant to the role</li>\n<li>Excellent working knowledge of rehabilitation needs including children and adults with sensory needs</li>\n<li>Confident in working closely with people, colleagues and autonomously managing a diverse caseload, meetings and lead on projects.</li>\n<li> Hold a small caseload of complex cases.</li>\n<li>Driving licence to travel county-wide for meetings and community visits</li>\n</ul><strong>Desirable</strong><ul>\n<li>Supervision experience</li>\n<li>Familiar with contributing to safeguarding practices</li>\n<li>Experiences of mentoring and coaching</li>\n<li>Specialist Project lead</li>\n<li>Willingness to learn and develop management and leadership skills, appropriate to the role.</li>\n</ul><br><br>This job role is within the Health and Social Care Services, level HSC12 job profile.
