Community-minded people needed for rewarding roles

  • Health and Wellbeing Officer (there is more than one vacancy for this position)
  • Temporary contract, initially offer­ed for 15 months
  • Full-time 37 hours per week contracts are available, although part time job share contracts will be considered.

Living Well Waltham Forest is a new pilot project funded by Waltham Forest Clinical Commissioning Group (WF CCG) and London Borough of Waltham Forest. It aims to support residents to make small, positive changes to their lives and attain their goals in improving their lifestyle.

The Health and Wellbeing Officers will form a key role within the team providing regular contact, liaison and support planning to older people identified as meeting the criteria for the programme. The purpose is to offer non-medical support as part of an person centred plan, not to duplicate the services provided by other agencies, and to ensure that services and support are delivered in a personalised, coordinated, relevant and timely manner.

This pivotal role makes sure that an older person’s independence goals are identified and achieved, and acts as a vital link between the person and the various services that support them to regain their quality of life.

You will recruit, train and oversee a team of volunteers offering encouragement and support for individuals to self-manage and self-care using the best practice available.

You will offer a consistent supportive presence, avoid duplication of service provision by other agencies and provide support that is tailored to the individuals that you are supporting. You’ll be a self-motivated manager, with proven leadership experience. A willingness to work closely with external partners is essential as is a commitment to ensure the success of the project.

You will work as part of a friendly team covering Waltham Forest working with a range of partners, including social care, NHS services such as community nursing and GPs and other voluntary organisations. You will be the central contact point for liaison, coordination, support planning and monitoring.

All posts are subject to an enhanced DBS.

Training will be provided.
