Team Manager - Kinship and Post Order Support Team

We are excited to announce a new opportunity for a Social Care Officer to join our Newly established Medway’s Intensive Support Team (MIST) within Adult Social Care. This role presents a unique chance to make a significant impact on the lives of individuals facing multi-faceted complexities, providing hands-on support and interventions within the community including but not exclusive to acute settings, individual’s homes and numerous other settings. The successful applicant will be required to have experience within Mental Health, Learning Disabilities, alcohol and Drug use, Safeguarding, homelessness, and outreach work.As a Social Care officer in this dynamic team, you will utilize yourself as a tool to engage directly with clients, often in crisis situations, and those that require a multi-disciplinary team (MDT) response. Your role will involve advocating for individuals, ensuring their needs are met, and facilitating access to vital services. Strong communication skills are essential, as you will be required to build and maintain relationships with individuals accessing support, their families, and partner agencies, including housing, police, probation services, mental health professionals, and addiction support organizations.Knowledge of the Medway area is crucial, as you will be navigating local resources and voluntary services to enhance the support available to individuals.
