We refer to the Reablement service and complete assessments of ongoing need. We triage all Safeguarding alerts and make the decision regarding whether an alert progress to a Section 42 enquiry.<br><br>The Hospital Team facilitates discharges from both Kingston Hospital and out of borough hospitals. This is completed using Discharge to Assess.
