Using a person-centred approach, you will look after the physical, emotional, culture and social needs of service users, which will involve personal care assisting service users with their meals and drinks and all aspects of daily living. Part of your role will involve activity planning and provision, thus promoting individual mental health and wellbeing. You will be key worker to specific individuals taking responsibility to record and communicate to the assistant managers to ensure all care plans are updated and current. This role will also include driving and escorting service users to and from the unit and out on planned trips. You will be looking after the needs of a diverse group of service users with a wide range of care needs and this will include some moving and handling and personal care duties. You will be expected to work as part of a team or on your own, liaise with multi-disciplinary professionals in respect of the needs of service users. Skills: •	Patience and an understanding of the importance of individuals dignity, privacy and choice when meeting the needs of older people who have a diagnosis of dementia / cognitive impairment. •	Ideally basic knowledge of IT systems or be prepared to learn so as to enable yo to update records access payslips and use e-learning. •	Flexible and adaptable to be able to take part in escort duties to meet and respond to the operational needs of the service. •	Good written skills and the ability to monitor, recognise and to accurately record service user’s information and needs for use in multi-agency reviews. •	Understanding of the cultural and communication needs of people from a diverse range of backgrounds. •	Able to work as part of a team. •	Good communication and be able to interact well with service users, the team, and other professionals. •	Able to deal with emergencies in a calm manner. •	An understanding of service user confidentiality and Data Protection and being able to follow policy and procedure such as medication administration, health and safety and strategies for working with people with challenging behaviour. •	A full driving licence. D1 category desirable but not essential, and the ability to pass the MIDAS minibus test this is desirable not essential. •	The ability to converse at ease with customers and provide advice in accurate spoken English is essential for the post. •	The ability and skills to be trained and competent in the administration of medication •	Have experience in a caring role as a formal paid or unpaid carer. •	You will be required to take part in training (on and off site) to meet the core training requirements and to gain level 2/3 health and social care certificate.

As a national trailblazer, we have launched our new ND pathway and Multidisciplinary 0-19yr Neurodiversity Service. This brings together experts from a range of fields to provide universal and targeted information, advice and guidance.The Role This exciting new role will give you the opportunity to work alongside a range of ND expertise in the city. Working closely with colleagues, children and their families you will help us to better understand the challenges they face when children have additional needs and help shape our work to meet these needs. You will deliver a range of information, advice and guidance for children, families and practitioners, and you will be central to improving the way in which we as a city support children, families and practitioners to navigate the many challenges and successes of being neurodiverse. You will ensure that practitioners across the workforce are able to map a child's individual profile and plan to meet their needs. You will ensure information, advice and guidance is provided to children, families and practitioners in the most effective way to ensure that improved outcomes are achieved, including reducing demand on high-cost specialist services. There will be occasional evening and out of hours working.The ideal candidateYou will need to have/be <ul>
<li>A nationally recognised qualification such NVQ level 4 or other higher-level qualification orequivalent experience. An up to date working knowledge of safeguarding children and young adults</li>
<li>Experience of working with young people and their families and have a positive attitude to helping young adults and delivering a safe and effective offer.</li>
<li>Experience of delivering training for parents and professionals would be an advantage.</li>
<li>You need to have an up-to-date working knowledge of neurodiversity and, the impact it can have on children and families.</li>
<li>A car driver with access to transport or able to easily access public transport to move across Portsmouth City.</li>
