<h2>About the role</h2>At Beaconsfield Villas we provide 24 hours, 7 day a week support for 6 adults who have a learning disability. &nbsp;The service users live in 3 separate flats that creates a homely environment and our dedicated team of staff work hard to ensure that our residents’ specific needs (e.g. Autism and Challenging Behaviour) are met in a caring and sensitive manner and skills are developed in a way that promotes their independence. &nbsp;Our last CQC inspection rated the service as “good” noting that family and carers were very happy with the staff team and the service that is provided to their family memberYou will join a large team of committed staff in providing high level support to those living at Beaconsfield Villas. The help you provide will be tailored to each individual’s needs and is designed to help them to maintain as much independence as possible.The work will involve:&nbsp;<ul>

Introduction This is an opportunity to join our management team as a Care Support Manager to help us provide Re-ablement Care across the city to people in their homes.&nbsp;
About us Independence at Home provides a personal approach to helping individuals who find themselves unable to manage day-to-day tasks at home. We support people for up to six weeks to learn to do things in a different way to increase independence and confidence. This includes support with daily living tasks including manual handling. Our service is integrated with Health, so we work closely with Nurses, OT and Physio-therapists. The service is part of the hospital discharge pathway, so we are very busy and we have high turn-over of service users. Our last CQC inspection rated the service as “good” in all aspects of the care and support provided, as well as for leadership.
<h2>About the role</h2>About the role<ul>
<li>Leading and managing a team of Home Care Support Workers working in the community providing care;</li>
<li>Setting up person-centred care for service users, and ensuring they receive an effective, appropriate service that enables them to achieve and maintain independence, with dignity and choice.&nbsp;</li>
<li>Reviewing service users’ needs after a period of re-ablement care and either discharging them, or, if necessary, organizing on-going, long-term care.</li>
