An opportunity has arisen for a Care Manager for 30 hours per week within the enhanced duty team of the Specialist Older Mental Health Service (SOAMHS) based at East Brighton Community Mental Health Centre. The post holder will be employed by Brighton and Hove City Council but will be co-located within Sussex Partnership NHS Foundation Trust (SPFT) as part of an integrated service. The operating hours of the service are 9-7 Monday to Sunday. There may be a requirement to work out of hours and at weekends on a rota system for which enhancements will be paid.&nbsp;

<h2>About the role</h2>The enhanced duty team offer intensive assessment and support to people with mental health needs. It is a multi-disciplinary team and is part of the wider SOAMHS service. &nbsp;There may be an expectation for you to undertake work as part of the SOAMHS team.&nbsp;We are looking for someone who has high professional standards with an excellent knowledge of current legislation such as the Care Act and Mental Capacity Act. Introduction This is an opportunity to join our management team as a Care Support Manager to help us provide Re-ablement Care across the city to people in their homes.&nbsp;
About us Independence at Home provides a personal approach to helping individuals who find themselves unable to manage day-to-day tasks at home. We support people for up to six weeks to learn to do things in a different way to increase independence and confidence. This includes support with daily living tasks including manual handling. Our service is integrated with Health, so we work closely with Nurses, OT and Physio-therapists. The service is part of the hospital discharge pathway, so we are very busy and we have high turn-over of service users. Our last CQC inspection rated the service as “good” in all aspects of the care and support provided, as well as for leadership.
<h2>About the role</h2>About the role<ul>
<li>Leading and managing a team of Home Care Support Workers working in the community providing care;</li>
<li>Setting up person-centred care for service users, and ensuring they receive an effective, appropriate service that enables them to achieve and maintain independence, with dignity and choice.&nbsp;</li>
<li>Reviewing service users’ needs after a period of re-ablement care and either discharging them, or, if necessary, organizing on-going, long-term care.</li>
<li>Helping to lead the service to ensure we maintain a quality service that meets CQC regulations and provides value for money.</li>
</ul>About you We will provide you with in-depth induction to the role, where you will be well supported by your managers and colleagues. You will also have access to training and continuous professional development appropriate to the role. However, you will need:<ul>
