As a Leaving Care Personal Adviser, you will develop an effective working relationship with young people eligible for leaving care services. You will motivate and effectively support them to engage in education, training and employment whilst ensuring they effectively handle their finances, live in suitable housing, and have the necessary emotional resilience and skills for independence.
As a Leaving Care Personal Adviser, you will need a wide range of skills including knowledge of issues that impact care leavers and unaccompanied young people. You will play a key part in supporting them into adulthood and preparing them with skills to ensure they have the best chances moving forward independently.
You will need to think creatively and take the lead on some participation and group activities for the cohort of care leavers that we work with.

The Mental Health Social Care Team work
 with adults with a diagnosis of functional mental
 disorder. 
 
We use the Conversation Counts model and the team offer the full
 range of social care interventions for the client group, from
 advice and signposting, adult safeguarding, discharges from the
 psychiatric hospital, to initial assessments under the Care Act
 and commissioning of services to meet eligible needs or mental
 health aftercare needs under S117 Mental Health Act 1989 and
 onward reviews. 
 
Good knowledge of the Mental Capacity Act is pivotal to the role
 and experience of court work and Community DOLS is required. We
 welcome applications for AMHPs. The team takes a strengths based
 approach and aims to help individuals tap into support available
 in their local network where possible with an aim of preventing
 needs arising and promoting independence in their own
 home. 
 
We benefit from a mental health reablement service which supports
 our clients to meet their goals and retain independence and
 recover from periods of ill health. 
 
Have a particular focus on reviewing and
 S117 aftercare, you will support the team manager with quality
 assurance of S117 applications across the council and the process
 and we are particularly looking for someone highly organised and
 efficient.
 
You will hold a small varied caseload as well as providing
 supervision, guidance and oversight to a group of social care
 practitioners, operating as the duty manager for the team on a
 rota basis and deputising for the Team Manager as necessary.&nbsp; You

You must have a current Social Work England registration and at least three years post qualifying experience within children’s social care that includes child protection, public law outline and care proceedings.

Senior Social Worker - Duty and Assessment

We are a Local Authority working for the community, improving the lives of our residents and we’re looking for a motivated and talented Senior Social Worker to join our Advice and Wellbeing Hub – SW Pathway.

As a Senior Social Worker, you will play a key role in delivering high-quality advice, early intervention, and support to those accessing the hub. You will provide professional leadership, undertake complex assessments, and work collaboratively with partner agencies to promote wellbeing and safeguard vulnerable individuals. &nbsp;
The Advice and Wellbeing Hub comprises a Social Work and OT pathways.&nbsp; The team takes all the new referrals into Adult Social Care for people 18 years upwards.&nbsp; It is a busy, supportive and friendly team, working with people of all ages and backgrounds and their families and carers. We use a Conversations Count strengths-based model to promote choice and control and achieve excellent outcomes. You will work with individuals who have been referred for support for up to 6 weeks, to maximise their independence in their own homes. Promoting choice and control and using a strength-based approach.

About the Role:
Your duties as Senior Social Worker include providing supervision to the Social Workers and Social Care Coordinators within the team and to undertake joint visits with other members of the team to support with knowledge sharing and skills development. You will also support the Team Manager in some aspects of managing the team as a whole and service development
The role involves you undertaking work using the Conversation Counts model, working on prevention and early support. Using your social work expertise, you will conduct Care Act assessments, safeguarding investigations, and Mental Capacity assessments, as well as facilitate best interest meetings. 
This role involves working with adults with diverse needs, including individuals with learning disabilities, physical disabilities, older adults, and those affected by substance or alcohol misuse. Additionally, you will provide information, advice, and support to carers, helping them navigate available resources and services. This position offers an excellent opportunity to develop your skills and experience in Adult Social Care, delivering high-quality, person-centred interventions to individuals in their own homes. You will be part of a friendly and supportive team committed to making a meaningful difference in people’s lives.

We have an exciting opportunity available for a Senior Social Worker / Approved Mental Health Professional (AMHP) to join our Integrated Mental Health Service, here at Wokingham Borough Council, on a full time, permanent basis.
As a Senior Social Worker / Approved Mental Health Professional (AMHP) you will join a passionate and dynamic team, playing an integral role as, we collaborate with The Health Trust to transform our services and develop a dedicated social care pathway.
Both Wokingham Borough Council and The Health Trust recognise the benefits of integration, which include holistic support for people accessing the service, peer support and working collaboratively on casework for CMHT (Community Mental Health Team) colleagues. Although WBC staff will manage social care and BHFT will provide health care separately, the service will collaborate closely as part of a multidisciplinary team to ensure the best outcomes for service users.&nbsp;
Wokingham CMHT is committed to professional development and has excellent working relationships with health colleagues and colleagues in Adult Social Care.
We have a number of service development initiatives coming up, including development of a hospital discharge team and an Occupational Therapy-led reablement service so it’s an exciting time to join us.

Key responsibilities:

<li>Hold a mental health caseload, providing care and services for people who have severe and /or enduring mental health difficulties.</li>
<li>Produce care plans and risk management plans and ensure their effective implementation.</li>
<li>Act in a supervisory role to other team members and provide supervision in line with the CMHT Joint Supervision Policy.</li>
<li>Identify safeguarding adult and children concerns and implement safeguarding policies and processes.</li>
<li>Maintain high standards of quality in clinical recording, ensuring information is recorded accurately, appropriately and kept up to date.</li>
<li>Carry out a reviewing function with people in long term stable placements who have care packages in place.</li>
