<h4>About the role</h4><h4>Role Summary</h4><p>To fully action and coordinate the delivery of Disabled Facilities Grants and other home adaptations and provide information and guidance to promote independent living. By conducting customer visits and completing any necessary grant or loan documentation required. This role supports our customers throughout the grant and adaptation process, from initial contact to completion. Together, we deliver essential home adaptations through the Disabled Facilities Grants awarded by Hastings and Rother councils.</p><p>The work we do helps people regain independence and stay safely in their own homes. Whether it’s installing a stairlift, a level access shower, or a ramp, every adaptation makes a real difference. Being involved in that process and knowing a customer has finally received something they’ve been longing for is incredibly rewarding.</p><p>Dawn Jellis, HASS Team Manager</p><h4>What will you be doing?</h4><h4>Role Specific Responsibilities</h4><ul><li> Be responsible for, and integral to the whole grant, loan or private works process and will assume full responsibility for the delivery of the service in both a visiting and administrative capacity. </li><li> Clearly explain the remit of the service to a range of customers and other professionals as appropriate. </li><li> Hold a caseload of customers and provide them with support in either their own home, or at an alternative venue if required. </li><li> Assess circumstances of ownership, and income to advise on appropriate welfare benefits and funding for works required. </li><li> Maintain timely administration of customer files as required by the service, and to enable the preparation of reports and statistics. </li><li> Undertake key administrative aspects and processing of grant and loan paperwork. </li><li> Accurately input customer data onto a database in a timely manner, ensuring compliance with the service specification, commissioner requirements and key performance indicators. </li><li> Ensure that notes about the customers are factual and free from personal opinions. </li><li> Deal with all enquiries in a way which promotes the service. </li><li> Act as the central liaison/keyworker for customers who are within the grant or loan process. </li><li> Liaise with external bodies, attend case review, and update meetings, as required. </li><li> Assist with processing invoices, and undertake any other financial responsibilities as required by the role. </li><li> Liaise promptly with colleagues, partner agencies and other stakeholders regarding safeguarding, welfare, and risk concerns in accordance with Town &amp; Country’s policy. </li><li> Participate in team meetings. </li><li> Undertake such other duties, training and / or hours of work as may be reasonably required, and which are consistent with the general level of responsibility for this role. </li><li> Ensure that all duties are carried out to the highest standard, and in accordance with policies and procedures, with reference to the health and safety and diversity and inclusion policy. </li><li> All staff working for Town &amp; Country have a statutory duty of confidentiality to protect customers and any personally identifiable information only use it for the purposes for which it was intended. </li><li> Work in collaboration with other departments to ensure a seamless service. </li><li> Undertake such other duties, training and / or hours of work as may be reasonably required, and which are consistent with the general level of responsibility of this job. </li></ul><h4>General</h4><ul><li>To adhere to the equal opportunities policy and to actively promote equality of opportunity wherever possible. </li><li> Recognise, respect, and promote the different roles and diversity of the individuals within the business. </li><li> To actively contribute towards key performance indicators and professional standards. </li><li> Work in accordance with the General Data Protection Regulations (GDPR) and Data Protection Law and be responsible for the integrity of personal information you process.
