£25,951 - £32,486 pa + annual retention payment of £4,000
Permanent Contract – 37 hours per week

The 0-25 Together Service has been operating for around five months and it has been an exciting time as we develop this service to ensure a seamless provision for children, young people and young adults.

We currently have opportunities in our 0-25 Together Service for experienced social workers who have worked in an adult social worker role. You will be responsible for assessing and managing support to people with physical and learning disabilities, as well as citizens who have Autism and their carers. You will have recent experience of managing complex case work in the areas of safeguarding and personalisation, and you know that with this diverse client group, no two days will be the same.

You need to know that we are a service motivated by and dedicated to effective social work practice. Our core aim is to support individuals to be an influential and active member of their community and promote true citizenship. Our practitioners strive to engage people, providers and promote inclusive community options as well as self-directed purchased services. We want our Social Workers to be resilient and robust as well as imaginative and reflective.
