An exciting opportunity has arisen to recruit a Transition Social Worker to the Community Team for People with Learning Disabilities (CTPLD) in North Somerset. The position is available for 37hours per week on permanent basis based in Clevedon and supporting the surrounding areas. We are seeking applicants who hold a Social Work Qualification with experience of working with Adults and Children - preferably within a community setting. Applicants need to be registered with HCPC, ideally with experience in Care Management (Assessment and Review), Safeguarding Adults, EHC Plans and Mental Capacity Act processes. This is an exciting and challenging time for Adult Social Care Services with the implementation of the Care Act 2014 and integrated working with our Community Health partners. The CTPLD is a multi-disciplinary team whose members include Reviewing Officers, Psychologists, Psychiatrists, Community Nurses, Physiotherapists, Occupational Therapists and the Intensive Support Team as well as hosting Transitions, a Housing Officer and the Shared Lives Scheme. We are supportive of the need for ongoing CPD as well as offering annual leave provision of 26 days per annum with this rising to 31 days per annum after 5 years of continuous service. This post is exempt under the provisions of the Rehabilitations of Offenders Act (Exceptions) Order 1975 and is subject to an enhanced Disclosure and Barring Service check.
