This has meant our team work with people in the acute setting as well as those in community settings after a period of time in hospital. This can be either in step down beds whilst the person is benefiting from rehab or in the person’s home. We also work with those who need support to prevent a hospital admission including playing a role at the front door of the hospital by working with our health colleagues in the Emergency Department.We are a friendly and supportive team working in a busy environment; although working under pressure we maintain high professional standards of practice and are committed to ongoing professional development. The successful candidate will be expected to work across all settings and develop and maintain good relationships with key partners.&nbsp;This is an exciting opportunity to join the team which is made up of Operations Managers, Senior Social Workers, Social Workers, and Social Care Workers. The team works closely with system partners including University Hospitals Sussex, Sussex Partnership Foundation Trust and Sussex Community Foundation Trust, so effective multi-disciplinary working is essential. &nbsp;The candidate would need to have a strong leadership style in the social work field, have excellent communication skills and be solution focused under pressure and time constraints.
