An opportunity has arisen in the East Kent District of Thanet. &nbsp;We are looking for a Senior Practitioner to join and guide a superb team of social workers in our children’s social work service.
The successful applicant will be supporting a team of Social Workers providing high quality social work in the service to Children in Need and Children subject to Child Protection plans.
Your role as a Senior Practitioner requires you to be an experienced Social Worker who is able to manage a caseload of complex cases involving assessment, planning, implementation and evaluation of appropriate actions. &nbsp;Ensuring resources are utilised effectively to safeguard and promote the welfare of children and their families.
You will also be required to contribute to raising and maintaining standards of professional social work within a social work team, develop areas of specialist practice, offer expert advice to less experienced staff, and act as a Practice Assessor for students and ASYE. You will be well motivated to support and understudy for your Team Manager in order to sustain and improve our performance.
Above all, you’ll be focused on helping children and families to change their lives for the better. You will be required to understand and help lead the delivery on the Change for Kent Programme in the Ramsgate Service, working closely with Early Help and Open Access services to deliver and shape Integrated Children’s Services.

Team Manager - Assessment

We are seeking a Qualified Social Worker to join the 16+ Service to primarily support unaccompanied asylum-seeking children (UASC) who are children in care to the Local Authority.Applicants will need to have good knowledge and experience of working with, assessing and supporting UASC who are therefore children in care, with full understanding of the Age Assessment, Home Office, Asylum and Children in Care processes and legislations. Scheme Support Officers play a crucial role in enabling residents to live independently, whether or not they have a care package, and in helping them maintain their tenancies.They promote resident’s wellbeing through regular welfare checks (daily or as agreed with the resident) and effectively collaborate with care management, community and nursing staff, the Medway helpline, GPs, and other organisations or stakeholders. Additionally, Scheme Support Officers are responsible for managing and maintaining the schemes, as well as monitoring and reporting on contractor performance.Liaise with stakeholders in a way that promotes the One Medway Council Plan and embeds our values and behaviours. Accountabilities and outcomes: To undertake tenant document reviews to ensure all relevant information is current and reviewed twice yearly (medical/health information, medication, GP, emergency contact details and information held by central control). Undertake routine checks and effectiveness of the call system, telecare devices, and fire alarms – and any other systems that are identified as contributing to the health, safety, and wellbeing of tenants. To undertake Fire, Health and Safety Risk Assessments and ensure all control measure are in place and applied, as to enable fire drills and evacuation procedures in accordance with the relevant risk assessments and procedures. Medway Council Job Profile To carry out needs assessment visits to include person centered risk assessment of all potential tenants to ensure they are suitable and understand the implications of moving into a HFIL sheltered scheme. Develop and maintain effective communication, engaging with residents to avoid social isolation and encourage them to participate in various activities. Proactively identify routine repairs and maintenance issues whilst carrying out daily checks at the schemes to promote a safe and healthy living conditions. To monitor the scheme cleaning, grounds maintenance and contractors carrying out their duties at the HFIL schemes. To ensure records are maintained to meet statutory requirements to support the efficient management and administration of the schemes and the welfare of residents and the provision of performance information. Scheme Support Officer

The Medway 16+ Care Leaver Service works to advise, support and befriend young people from the age of 16 who are in care and those young people who have left care (up to the age of 25) with a diverse range of complex needs to enable them to manage the transition into adulthood and independence.The Specialist Housing Advisor will support Personal Advisors and Care Leavers within the 16+ service, who require additional support to address their accommodation and independent living needs, with a focus on those at risk of homelessness and those ready to move onto independent accommodation.The role will require the post holder to provide assistance, advice, encouragement and support to Care Leavers to ensure they actively participate and contribute to their Housing Plans / Pathway Plans to enable them to move successfully into independent living. This role will focus on those Care Leavers at high risk of homelessness, due to inactivity, vulnerability or other factors. The Specialist Housing Advisor will be expected to keep up-to-date with government guidance and regulations in relation to accommodation, as well as all involved processes and resources, to facilitate the delivery of expert advice on all areas of in independent accommodation.
