What will you be doing?Family Hubs are part of Kent County Council’s offer to support to children and young people, aged 11-19 (SEND 25) and their families.Our local Family Hubs offer a broad and integrated range of early help services for families, which include midwifery, health visiting, parenting support and activities for children and young people.We have 56 Family Hubs across Kent, where we operate alongside our partners to support local communities.An exciting opportunity has arisen to join the newly formed Family Hub team in Gravesham District. The successful candidate will operate as a Family Hub Practitioner within the Family Hub team in an agile fashion at a variety of outreach settings (including home visiting) within a district providing 1:1 and group support to children, young people and their families. The service sits alongside the Adolescent service working with complex families in a trauma informed approach working to build trusting relationships to enable behaviour change.&nbsp;The team is small but benefits from a range of specialist workers which will support the social worker to work with the child and family.
