Older Peoples Service - West

Dacorum Team

The Older Person Service is a dynamic service that works across the community under the Care Act to safeguard, assess, plan support and review adults and cares with long term health and social care needs. We can offer Hybrid working arrangements, flexible working practices, community engagement and volunteer activities and, as part of our commitment to you, as well as promote your continuous Learning & Development.


Adult Disability Service

Stevenage Team
37 hours per week | Fixed Term to January 2027

Welwyn & Hatfield Team
37 hours per week | Fixed Term to Aug 2027

North Herts Team
37 hours per week | Permanent

The Adult Disability Service provides support to adults living with a learning or physical disability. We aim to enable people to make positive decisions in their lives, promote their independence and ensure they can get involved in their local community. We're a service motivated by and dedicated to social care intervention and community social work.


Older Peoples Team East

North Herts Locality
Farnham House, Stevenage
37 hours per week | Permanent and Fixed Term to March 2027

The Older Person Service is a dynamic service that works across the community under the Care Act to safeguard, assess, plan support and review adults and cares with long term health and social care needs. We have a range of teams within Older People services, including locality based teams, both residential and community reviewing teams, crisis intervention and prevention of hospital admission teams, and sensory services. We can offer Hybrid working arrangements, flexible working practices, community engagement and volunteer activities and, as part of our commitment to you, as well as promote your continuous Learning & Development.


Older Peoples Team West - Prevention of Admissions Team
Apsley, Hemel Hempstead
37 hours per week | Permanent

The South West Prevention of Admission Team supports individuals who live in their own home or usual place of residence and are aged 65 or over, or under 65 with a confirmed dementia diagnosis, who are experiencing a deterioration in health or care circumstances that is likely to lead to a hospital, residential, or inpatient admission within 48 hours.

The team covers South and West Hertfordshire, including the localities of St Albans, Dacorum, Watford, Hertsmere, and Three Rivers.

About you

Essential:

• Social Work degree
• Registration with Social Work England
• Full Driving licence

Driving required
