Job Description

Salary range: Grade 6 £35,520£36,585 p.a. plus fantastic benefits
Contract: Fixed Term - 31 March 2027
Hours of work: 36 hours per week

Are you ready to take on a rewarding challenge? We’re seeking a skilled and committed Health and Wellbeing Assessor to join our LIFE (Living Independently For Everyone) service. This role plays a vital part in supporting individuals to regain independence and live confidently at home.

As a trusted assessor, you’ll work alongside Social Workers, Occupational Therapists, Physiotherapists, and Intermediate Care Nurses.

You’ll work autonomously across a range of settings, including hospitals, community locations, and people’s homes delivering person-centred support that empowers individuals to reach their full potential.
