The Role:

We’re looking for an enthusiastic, dynamic, and compassionate Social Care Worker to join our friendly and supportive Reablement Team – a service dedicated to empowering people to rebuild their confidence, skills, and independence.

About the Role

As part of our Reablement Team, you’ll play a vital role in transforming lives. You will provide short-term, intensive support to individuals following hospital discharge or community referrals, helping them regain independence and stay in their own homes for as long as possible.

This role is about enabling, encouraging, and inspiring people to achieve what matters most to them.

What You’ll Be Doing

  • Delivering short-term reablement support to help individuals regain daily living skills and independence
  • Supporting people in their own homes and communities to achieve independence
  • Undertaking needs assessments to ensure care and support is tailored to each individual
  • Undertaking Mental Capacity Assessments in line with legislation and best practice
  • Completing Continuing Healthcare (CHC) checklists to support eligibility decision-making
  • Liaising with the Neighbourhood Team to ensure joined-up, coordinated care and support
  • Focusing on strengths and abilities – what people can do, not what they can’t
  • Working creatively to find solutions that improve lives
  • Collaborating closely with health professionals, including Occupational Therapists, Physiotherapists, social workers, and community services
  • Helping individuals access community resources, activities, and support networks
  • Making a real, visible difference to people’s wellbeing every single day

About You

We’re looking for someone who is:

  • Passionate about making a difference
  • A confident communicator and team player
  • Creative, proactive, and solution-focused
  • Experienced in health or social care or holds a Level 2 qualification
