We are a long-term generic adult team, working with people from 18 (or approaching 18 for transition cases) and their carers, covering the South of Amber Valley. The people we work with have social care needs resulting from a range of disabilities people with learning disability and autism, physical disability, and dementia. The successful candidate will have a working knowledge of the Care Act 2014 and Mental Capacity Act 2005. Using person centred, strengths-based approaches we support people to identify and achieve their goals with a home first approach. <h4>About the Role</h4>
We are seeking a dedicated and compassionate Recovery Worker to join our team which supports adult male residents living with mental health conditions, often alongside drug or alcohol dependences, within a residential two year supported living environment. Within the service, you will provide person-centred, flexible support to help residents achieve their goals, maintain stability, and reduce hospital admissions. You will play a key role in residents’ rehabilitation and reintegration into the community by delivering tailored interventions and working closely with internal teams, partners and external agencies to achieve positive outcomes.
