Social Worker Level 2 – West Community Wellbeing Team

We have two vacancies in our Community Wellbeing Team covering our Lincoln North & Gainsborough Team and one vacancy in our Review Team

  • Work closely with adults and families to provide tailored support and care planning in a strengths based and person centred way, embracing what community assets and strengths.
  • Champion independence, dignity, and wellbeing in every interaction
  • Collaborate with health professionals, community partners, and colleagues to deliver joined-up services
  • Bring compassion, resilience, and creativity to complex situations
  • Help develop our workforce through supervision of up to 4 Community Care Officers.

A Smarter Working Employer
