This is a zero-hours position to cover evening and weekend clinics.The post holder will be required to offer specialist support to smokers wanting to quit smoking through the one-to-one service currently offered to people living or working in Medway. The Health Improvement Coordinator will be responsible for facilitating each individual quit attempt, providing behavioural support, with practical advice, social support and pharmacotherapy advice. Strategically planning support sessions for each client and working to increase their motivation are essential components of supporting a successful quit attempt and so form a crucial part of the adviser role. The post holder must be able to collect, record and maintain accurate data in line with national guidance. It is vital that the post holder is an excellent team player and is flexible to be able to cover team member’s clinics on occasions such as annual leave or sickness absence. This role will require the post holder to be able to work regular evenings each week (generally no later than 8pm) and on weekends, according to the service needs. We’re looking for a dedicated and highly organised individual to join our team as an Education, Health and Care (EHC) Assessment Officer.About the RoleIn this vital role, you will be responsible for managing the Education, Health and Care Needs Assessment (EHCNA) process and producing high-quality Education, Health, and Care (EHC) plans within statutory timeframes. Your work will span all educational phases—from Early Years through to post-16—ensuring that each plan reflects the unique needs and aspirations of the child or young person.You’ll gather and analyse information from a range of sources and collaborate closely with professionals across education, health, and social care to identify and deliver meaningful outcomes.What You’ll Be Doing‒ Managing a large and varied caseload in a fast-paced environment.‒ Drafting clear, outcome-focused EHC plans based on comprehensive assessments.‒ Communicating regularly with parents/carers, education providers, and multi-agency professionals.‒ Using data sets to inform your daily work and decision-making.‒ Leading and representing Medway Council at key meetings, including co-production sessions, Ways Forward meetings, supporting tribunals team, and other operational discussions.‒ Ensuring compliance with all relevant legislation, including the Children and Families Act 2014 and the SEND Code of Practice (2015).What We are Looking For‒ Excellent organisational and time management skills.‒ Strong communication and interpersonal abilities.‒ A sound understanding of SEND legislation and statutory processes.‒ Confidence in working independently and collaboratively in a multi-agency environment.Additional Information‒ As a customer-facing role, applicants must meet the language requirements outlined in Part 7 of the Immigration Act 2016, demonstrating a proficient command of spoken English/Welsh.This is a hybrid role, with office attendance required 3x per week.In return we will offer you: <ul><li>Generous annual leave entitlement</li><li>Generous local government pension scheme package</li><li>Access to wider training and development qualifications</li><li>Flexible working scheme</li><li>Travel plan discounts</li><li>Access to CAREFIRST advice &amp; counselling service</li><li>Employee discount scheme for national and local retailers</li></ul>Career Progression Frameworks (CPF) have been introduced for all roles at Medway Council. These frameworks provide a mechanism for employees to take ownership of their development within their post and be rewarded accordingly. This means that the salary you will be offered will be at level A unless, based on an assessment against the CPF criteria, you demonstrate that you are meeting the criteria of level B or C.
