Community Care Officer

Job Title: Community Care Officer Integrated Hospital Discharge Service
Location: Apsley, Hemel Hempstead
Salary: £22,462 progressing to £29,636 (only applicable with relevant qualification)
Hours: 37 hours per week
Contract: Permanent

We are seeking an enthusiastic, dynamic and resilient Community Care Officer to join our Post Hospital Review team within our Integrated Hospital Discharge Service.

The Service supports an integrated approach to managing the care of people who require support upon discharge from hospital, so the role will involve working alongside occupational therapists, community care officers, health colleagues and care providers.

You will be required to review support packages of people discharged from hospital on our various transitional pathways, including carrying out assessment of need and setting up longer-term packages for those requiring on-going support.

You will have the ability to undertake innovative assessments and care planning with individuals, carers and their personal support networks. 

We promote personalisation focusing on the wellbeing of the individual.  Through care planning we strive for increased independence, choice and control by enable people to direct their own support.  You will gain extensive experience in the field of adult social care and multi-disciplinary working.

You should be able to demonstrate a commitment to supporting people with the right care and information at the right time to help prevent future need, enable independence and promote positive wellbeing that will empower people to live the lives they want to.

The post will require you to visit service users in the community to undertake reviews, so a driving licence would be advantageous as is the ability to respond to situations quickly and flexibly.

To improve outcomes for service users, the Integrated Discharge Service supports a 7 day Hospital Discharge service.  The post will be asked to be part of the service-wide rota to provide cover on consecutive Saturday or Sunday once every eight weeks and will receive a day off during the week after. There will be opportunities for you to discuss this at interview if your application is successful.
