Social Prescribing Link Worker

Rutland Primary Care Network and Rutland County Council are working together to form a team who will work with people registered with the four Rutland GP practices to support them to achieve holistic goals, and enable them to live the life they choose. 

You will work closely with Rutland GPs, health and social care professionals and voluntary organisations, to support people who may benefit from making changes to their lifestyles which have far reaching effects on their health and well-being. This is a chance to shape a new role, as part of our award winning teams who are leading the way in health and social care integration, ensuring the services we deliver meet the needs of our service users in a fully personalised and joined up way. With continually evolving services, this is a great opportunity to join a nationally recognised and committed team, supporting service users and their carers every step of the way. 

This is an exciting opportunity to work as part of a multidisciplinary, integrated team. You will empower people to manage their health conditions and focus on what matters to them, through creating support plans in partnership with service users and carers. Taking a holistic approach, you will connect people to community groups and statutory services for practical and emotional support. 

What we need from you…

  • Knowledge of relevant legislation and recent developments in social care, and working knowledge of vulnerable individuals within the statutory, independent or voluntary sector
  • Ability to undertake social care assessments, provide outcome focussed support plans and monitor and review outcomes
  • Excellent communication skills, with a strong ability for partnership working
  • Great team working skills and ability to organise and prioritise your own workload
  • Flexible approach to your working pattern, and mobile and able to work from various locations

The successful applicant for this post will be subject to an enhanced check by the Disclosure and Barring Service (DBS) – costs for this are funded by the organisation.
