The London Borough of Islington is looking to recruit an individual to the Supported Accommodation Referrals team to work on referrals, placements and move on for vulnerable adults with offending histories through the supported housing pathways.
The team sources supported accommodation placements for vulnerable clients. The purpose of such support is to enable clients to live as independently as possible. You will be responsible for managing effective pathways into supported housing and advising on alternative housing options.
You will liaise with Housing Managers, Care Managers/Brokers, substance use services, mental health services, the National Probation Service and local CRC team as well as the voluntary sector to ensure vulnerable people are appropriately housed and supported accommodation services are fully utilised. The post holder will also conduct periodic housing assessments and provide advice and assistance on a range of housing options including tenancy sustainment.
You will be responsible for representing the Housing Department at key meetings such as Mappa, IOM and facilitating and attending case conferences as the need arises.
You will have experience of working with vulnerable people within a housing setting and possess the skills necessary for successful inter agency working.
The successful Candidate will be required to undertake a Debarring and Disclosure Service check.
