We are seeking an Area Manager to support our busy acute Hospitals in Lincolnshire and bordering counties, as well as supporting our Customer Service Centre and Community Wellbeing service. The post holder must hold a professional Social Work, Nurse, Occupational Therapist or Physiotherapist qualification. Registration with the HCPC, Social Work England or NMC must be maintained and renewed appropriately.

As an experienced leader you will provide strategic vision and be able to inspire, motivate and promote excellence within our acute hospital based teams. You will champion the principles of Social Work and be able to demonstrate values that promote wellbeing, personalisation, challenge oppression and discrimination, and empower people to live well.

The post holder will ensure timely integrated discharge pathways are maintained and improved. The post holder will need considerable experience of working in Adult Social Care and a hospital environment. The ability to negotiate and appropriately challenge at a senior level; together with good interpersonal and communication skills is essential.

The successful applicant will have extensive knowledge of the Care Act 2014, Mental Capacity Act 2005 and of the Delayed Transfer of Care (DTOC) process. Working collaboratively with our system partners you will support the flow of service users through the acute hospitals and ensure Better Care Fund (BCF) targets are achieved.

The post holder must be flexible, adaptable and able to work autonomously in pressured environments.  You will have the ability to manage and prioritise conflicting demands from multiple systems and be politically aware. You will have excellent time management and organisational skills and the ability to meet tight deadlines.

Although the role is based at Lincoln the successful person will be required to travel county wide to support all hospitals within their responsibility.
