Support Outreach Worker

We are seeking a caring and committed individual to join our Day Opportunities team. You will support our Resource Centre Manager and work with our team of Care Practitioners to deliver personalised services to our customers with learning disabilities, physical disabilities and those who may be frail or have dementia.

You will be responsible for organising and leading on activity and occupational sessions at Greenfield House Resource Centre and supporting our customers to access these sessions, experience of working with and supporting adults with disabilities is therefore essential.  This role also involves working with customers on an individual basis in the community on specific short term pieces of outreach work, so the ability to drive is also essential to this position.

This role has been identified as public facing in accordance with Part 7 of the Immigration Act 2017; the requirement to fulfil all spoken aspects of the role with confidence in English applies.
