Safe & Settled Officer (Hospital Discharges)

We are looking for a highly motivated and enthusiastic person to work with our Safe & Settled Co-ordinator. As a support officer you will be based in the community supporting patients recently discharged from hospital. Safe & Settled is a new service that will be based at the Royal Surrey County Hospital and will be managed by the Guildford & Waverley Care & Repair Team. The aim of the service is to help facilitate safe and timely discharges for older and vulnerable residents living in Guildford & Waverley. After a period of settling back home the Safe & Settled support officer will offer further support, sign posting patients to appropriate community services to enhance patient’s wellbeing and reduce loneliness and isolation.

Job Profile

What will be doing on a typical day 

  • Complete daily visits to patients - Prioritise as appropriate 
  • Follow discharge support plans 
  • Record case notes on each visit 
  • Ensure patient home environment are appropriate 
  • Referrals to Guildford & Waverley Borough council services 
  • Referrals to Voluntary sector 
  • Undertake wellbeing telephone calls for low need patients 
  • Review wellbeing calls with Co-ordinator and prioritise levels to high/moderate/low risk 
  • Report concerns to Co-ordinator if a patient presents unwell, not eaten or taken medication etc 
  • Liaise with patients and family members to offer advice and guidance 
  • Support the promotion of Safe & Settled in the community 
  • Knowledge and understanding of Surrey County Council safe guarding procedures and the skill to identify vulnerable adults 
  • Responsible for accurate recording of Safe & Settled outcomes 
  • Adhere to policies and procedures that underpin the implementation of the Safe & Settled service

 

At minimum you will need  

 

Experience from one of the following ranges of multi-agency statutory and voluntary employment backgrounds eg: Social Worker / Assistant, family support worker, health or social care reablement assistant, Health or Social Occupational Therapy assistant / Technical Instructor

Educated to Level 2/3 vocational training trusted assessor, or equivalent

               

  • Ability to take a practical approach to supporting older and vulnerable patients
  • Ability to carry out informal assessments in the home in partnership with families, using observational skills and having good problems solving skills
  • Ability to work effectively on own initiative, prioritise workload and meet deadlines
  • Willing to work outside normal office hours on occasions
  • Ability to travel extensively within the boroughs of Guildford & Waverley
  • An enhanced DBS disclosure and a Children and Adults barred list check will be required for this role
  • Car driver and access to own vehicle essential
