Salary Range: £26,999 - £38,813 pa + £1000 welcome payment

Older Peoples Services (Herts Valleys area based at Apsley, Hemel Hempstead) are looking for an experienced Social Worker who is enthusiastic, dynamic and resilient, to join Three Rivers Older People Services. You will need to be SWE registered, have successfully completed your ASYE and have knowledge and experience of relevant legislation in relation to social care, safeguarding, continuing health care and mental capacity. You will have the ability to undertake innovative assessments and care planning with individuals, carers and their personal support networks.

Older Peoples Services promote personalisation focusing on the wellbeing of the individual.  Through care planning we strive for increased independence, choice and control by enabling older people to direct their own support. We need a practitioner who can engage with older people by providing, promoting and enabling inclusive community options as well as self-directed purchased services. You will be a resilient, imaginative and reflective practitioner who can work in a busy environment.

This post requires you to visit service users in the community to undertake assessments and reviews, so a full UK driving licence is essential as is the ability to respond to situations quickly and flexibly.

You will have proven success in showing good communication skills in building relationships with service users; families; carers and professional colleagues in a multidisciplinary environment.
