Social Worker - Ongoing Support, R & R - Integrated Learning Disability Service

The Integrated Learning Disability Service seeks to achieve the very best outcomes for people with learning disabilities by supporting them to achieve their full potential and fulfil their aspirations. The service supports people with a learning disability across the London Borough of Hackney, as well as those in the City in relation to their health needs.

We are seeking an experienced Social Worker who is looking to take the next step in their career to join our growing team within the Hackney Integrated Learning Disability Service. You will join our dynamic, integrated multi-disciplinary community service which provides specialist health and social care services to adults with learning disabilities and complex needs in Hackney.

The Hackney Integrated Learning Disability Service (ILDS) is organised into multi-disciplinary care programmes (Preparing for Adulthood, Intensive Support, Ongoing Support, Referral & Reviews) providing a broad range of interventions.

As a Social Worker within Integrated Learning Disabilities Service, you will be key to providing quality social work to the people of Hackney. We will support you to achieve the very best outcomes for our residents.

You will join a newly formed team of social workers employed by London Borough of Hackney and health professionals employed by East London Foundation Trust. The service supports people across London Borough of Hackney as well as those in the City in relation to their health needs. It seeks to achieve the very best outcomes for our residents supporting them to achieve their full potential and fulfill their aspirations.

You will have experience of working with learning disabled people ideally in a multi-disciplinary setting.

A commitment to integrated working across health and social care is essential. 

You will be committed to person centred working, an ethos of co-production and working in our “Strengths Based Practice Approach”

You will have a commitment to your own ongoing social work education and be ready to develop your career through our academy approach. 

You will need to be Social Work qualified or equivalent and registered with HCPC. We welcome applications from people with a range of experience whether you are a newly qualified social worker or someone with many years of experience.

An enhanced DBS check is required.
