Senior Social Worker - Diabetes Team

A vacancy has arisen within the Diabetes Home Care team, based at the Birmingham Children’s Hospital, for a social worker who will support the specialist multi-disciplinary diabetes team in the on-going support, care and education of children and their families, following a diagnosis of diabetes. This post is an opportunity for a dedicated individual with experience in working with children to work alongside the existing team of 3 Clinical Nurse Specialists, 5 Diabetes Specialist Nurse’s, 3 specialist diabetes Dietitians, Family Support Worker and Youth Worker.

We are currently one of the largest Paediatric Diabetes units in the UK and deliver care to over 450 children with Type 1, Type 2 and rare diabetes. The team provides 24 hour a day, 7 day a week, outpatient and inpatient contact for families and you will be included in the Monday to Friday, day time shifts.   

The post holder will support Team members in providing care to children, young people and their families affected by diabetes. It is expected that the post holder will develop their knowledge and understanding of this condition and contribute to the day to day management of those patients and families. In particular, you will work closely with the diabetes family support worker and youth worker, to provide additional support to those who have social and economic difficulties, that impact on their ability to manage their condition.

You should be enthusiastic and self- motivated, with an interest in diabetes.  You will be expected to embrace change and be dedicated to supporting the provision of expert diabetes care through collaborative working with the multi-disciplinary teams in diabetes and throughout the Trust.
