Job Title: Social Worker, Three Rivers Extended Involvement Team
Location: Apsley, Hemel Hempstead
Salary Range: £27,741 - £39,880pa + £6,000 relocation package, £1000 welcome payment & lease car* terms apply plus 
Hours: 37 hours per week
Contract: Permanent

Work alongside some of the sector’s most passionate professionals, as a Hertfordshire County Council Social Worker/Senior Social Worker.

This is an exciting and rewarding role within our Older People Teams, to provide responses to those in need of support.

So what does this role involve?

You’ll need to be able to respond to complex needs, which can be difficult or ambiguous at times, and work closely with our carers. Your acute analytical skills will be used to inform assessment, make decisions, and intervene when needed – also supporting others to do the same.

You will also be able to ensure effective safeguarding and risk management, so that our people are protected from harm.

The role requires you to address adversity (such as poor health and poverty), as well as social exclusion, to help maximise strength of individuals and their communities. You will promote independence and autonomy with all who you work with, using your person-centred approach.

You will be an ambassador for our HCC Values, Behaviours, and Professional Standards, both internally and externally. This will also involve contributing to the learning and development of others. Alongside this, we will continually support you in your professional development.


  • Social Work degree, Social Work diploma, CQSW or CSS
  • Registered with Social Work England (SWE)


  • With adults and or children within a health and social care setting
    Involvement in multi-agency work
  • Applied knowledge of at least one of the following: Mental capacity Act; Mental
  • Health Act; Care Management; Continuing Health Care, Care Programme Approach
