Team - North Herts Older People Service

North Herts Older People Service cover a large rural geographical area including Hitchin, Letchworth, Baldock, Royston and Villages and provide support to older people and adults with dementia. We promote personalisation through our new assessment process focusing on the wellbeing of the individual. Through care planning we strive for increased independence, choice and control by offering personal budgets to enable people to direct their own support.
We are based at Farnham House, Stevenage however we encourage smart working with the benefit of a laptop or Ipad and smart phone, so you have the option of working at home or at one of our touchdowns across the area.
We are looking for experienced SWE registered Social Workers who will support with completing care needs assessment, developing innovative support plans, building networks and raising awareness in the area and developing valuable links for the team. You will work alongside Community Care Officers and Occupational Therapists to achieve the best outcomes for the individuals you work with.
