The Adult Disability Service provide support to adults with a learning or physical disability. You will be responsible for delivering high quality statutory social care including safeguarding, assessment and care management functions as well as developing innovative, creative community solutions. You will be pivotal in developing links with the local community through undertaking multi-agency work with partner organisations, providers, professionals, individuals, families and carers.

The role is part of an innovative and supportive service that strives to ensure that Hertfordshire’s people don’t just have their needs met but are listened to, supported, respected and included as citizens within our local communities.
We are based in Stevenage however we encourage smart working with the benefit of a laptop and smart phone, so you have the option of working at home or at one of our touchdowns across the area.
We are looking for a SW(E) registered Social Worker who will support with completing care needs assessment, developing innovative support plans, building networks and raising awareness in the area and developing valuable links for the team. You will work alongside Community Care Officers, Community Learning Disability Nurses and Occupational Therapists to achieve the best outcomes for the individuals you work with.
