Senior Social Worker

We are a friendly and dynamic newly-formed team who support 14-25 year olds with complex disabilities using creativity and forward-thinking. Our young people are at the centre of our assessments and we like to work with the person’s whole family/network, where possible, to transition the young person in to Adult Social Care.

We are looking for a senior social worker who can join us in supervising social workers and managing complex cases, creating robust, person-centred assessments and reviews for our young people.

Working closely with the Special Needs and Disabilities (SEND) team and also the Continuing Health Care pathway, we are in the lucky position of having multiple agencies to work alongside us to help implement the best plan of support we can for that young person which include schools, health and our own Mental Capacity/Best Interest Assessors.

This will include enjoying all the benefits of a small, close team who promote training and opportunity for every individual as well as regular, comprehensive supervision.

We provide support for young people with complex and significant disabilities. Aiding their transition around independence skills, education and work. Assessing for direct payment needs, and accessing the provision of supported living. As well as working with Special Educational Needs and Disabilities (SEND) and aiding Education Health Care Plan needs and Continuing Health Care requirements, around transition to adulthood needs and at times support for residential school. Supporting Children in Care to Transition to Adulthood as well as Special Educational Needs and Disabilities (SEND) Adult Social Care requests.
