Job Title: Community Care Officer
Location: Countywide with bases at Hemel Hempstead/Stevenage
Salary Range: H6-H8 £23,080 progressing to £30,451pa
Hours: 37 hours per week
Contract: Permanent

Job Duties:

This is an exciting opportunity to join the Sensory Services team based at Apsley Campus, Hemel Hempstead and Farnham House, Stevenage.

We are looking to recruit a Community Care Officer, who is enthusiastic, proactive and can build professional trusting relationships with service users, their carers and other organisations.  You will be confident in working closely with service users within the Sensory Services team and managing a diverse case load.

This is a great opportunity to work in our dynamic, multi-disciplinary and supportive team of 20, whose focus is on enabling individuals (children/ adults) who are deaf/ hard of hearing, have a visual impairment or are deafblind to achieve optimum control of their lives.

Using assessment tools and support plans you will work with service users and carers and carry out assessments in the person’s own home, therefore you will need to have a full UK Driving Licence.

You must have the ability to gather records and provide information electronically, and to implement care manage30,452ment. You will be a highly motivated individual who can support the community/district by advising on the best care/ support for people with sensory needs following your initial assessment. You will be comfortable working alone, visiting clients in their homes and developing professional relationships.

You will work as part of the team, have excellent communication skills (BSL and deafblind communication would be advantageous), and have a sound working knowledge of this service user group including people with other sensory needs who require social care support. 

You must have a sound grasp of social care legislation including safeguarding practices and the ability to work flexibility to meet service priorities. Working in partnership with other statutory agencies and community/ voluntary organisations, you will contribute to the development of services to improve customer journey experiences.

You will be supported by our committed and supportive team including professional supervision.
