Job Title: Assistant Service Finder - Community Solutions Team
Location: County wide (Predominantly based in either Apsley or Stevenage)
Salary Range: H6 £23,080- £25,481 pa
Hours: 37 hours per week
Contract: Fixed Term until March 2021
We currently have an opportunity for 3 Assistant Service Finders in the Community Solutions Team. You will have an office base in either Stevenage or Aspley however the post is county wide and will therefore involve travel to another site as required. The team are currently working from home.
The assistant service finder will be part of the Community Commissioning team for Adults with Disabilities (CCAD). Individuals may have a learning disability, physical disability, autism, Mental Health or a combination of needs.
The Assistant Service Finder will source and commission services, as directed by an adult social care assessment. This is the Micro commissioning function where services are sourced from providers that have agreed contractual terms and conditions, prices and process.
You will be responsible for sourcing services from contracted providers for;
- Hospital Discharges
- Short Breaks
- Day Services
- Community support
- Direct payments
You will need to be able to maintain accurate records, follow business processes and contribute towards process development.
It would be advantageous to have experience of working with people who have learning disability autism and/ or physical disability and a good understanding of what their support may look like.
You will need to develop good working relationships with our care providers and be able to build a thorough knowledge of the services they offer. You will be working closely with our Adult Disability Teams, 0 – 25 Together Service, hospital teams and work in partnership with Commissioners to ensure appropriate and effective provision is sourced and commissioned. Excellent IT, communication and organisational skills are essential as is the ability to work under pressure.
You will need to be able to travel thus a driving licence and access to a car is essential as is a willingness to work flexibly.
