Job Title: Advanced Practitioner
Location: Primary base – Farnham house (but will be able to work remotely)
Salary Range: M1-M2 £35, 745 - £43,857pa plus £1,000 welcome payment
Hours: 37 hours per week
We are seeking an enthusiastic, dynamic and resilient Advanced Practitioner to join our team on a permanent contract
The Welwyn & Hatfield team supports older adults to remain as independent as possible and supports an integrated approach to managing the care of people who require prevention of admission to hospital. You will need to be SWE registered and have a wide knowledge and experience of relevant legislation. You will inspire, lead, motivate, nurture and manage a team of social workers and CCO’s. The AP leads on peer development and learning in conjunction with development programmes and workforce strategy and provides coaching, mentoring and supervision as appropriate.
You will be responsible for ensuring the service provided is effective, efficient and delivers positive and personal outcomes for the citizens of Hertfordshire. Driving the performance and practice of the collective team and individuals is an integral part of the role, as too is the assistance to the Team manager and Deputy Team manager in the effective management of staffing, resources and budgets. In addition, you will provide expert information, advice and guidance in complex situations to your peers.
You will be able to demonstrate a commitment to supporting people with the right care and information at the right time to help prevent future need, enable independence and promote positive wellbeing that will empower people to live the lives they want to. You will take part in specific departmental projects. You will deputise for the Deputy Team Manager as required.
A driving licence would be advantageous as is the ability to respond to situations quickly and flexibly.
